Feel free to download and complete the following Admission forms prior to your initial visit. This will help save you some time. This paperwork contains information about Tisdale Holistic Therapy and Coaching, which includes acknowledgment forms, notice of privacy practices, payment obligations, and a few others. Copies of these forms are also available in our office if you are not able to print them out or if you prefer to fill them out upon your arrival for your first visit. If this is the case be aware of the extra time needed to go over and fill out the forms.
Additionally, please be sure to bring your drivers license or photo ID, insurance card(s) if you will be filing through your insurance, and any pertinent medical records or referrals for service.
